If I ever divorce and when I get to the point of dating again I am going to hand the next guy my new dating application. 😉

Feel free to add to it as you see fit….

1. Have you been in serious relationships, engaged, left at the altar, left someone at the altar, married, divorced or widowed? List how many times for each and for how long. Use back page if necessary for more room.

2. Do you have any biological or adopted children? Have you ever raised another woman’s children that you consider your own children? If you have children list ages. Do you have good relationships with them? If not, why? Do you pay child support on any of them or have you ever refused to pay or not been able to pay if so?

3. Do you have debt? List credit score and attach credit report as well.

4. Are your parents living? If so, do you get along with them and are they still married? If you don’t get along with one or both state the reason. If deceased, list dates of death and causes.

5. Do you have or ever had any sexually transmitted diseases? If so how did you acquire them? Use back page if more room is needed to explain.

6. Have you ever cheated on a significant other or had it done to you? If you did, list reasons why. Use back page if necessary for more room to explain.

7. How long have you had your job? If less than 10 yrs do you job hop? Ever been fired from a job? List reasons

8. Do you use alcohol or drugs? If so, which types and how often?

9. Are you a sex addict? Have you ever been accused by a SO of being one? Did you seek treatment? List which kind you are. Use back page if necessary for more room to explain.

10. Has alcohol, drugs or sex addiction ended any relationships or jobs you had? List all here.

11. Do you believe it’s ok for men, as well as yourself, to watch porn or read erotic magazines or books because it’s something all guys normally do?

12. Is any person ever justified in having sex outside of a committed relationship or marriage?

13. Have you ever been arrested? If so for what? Did you receive probation or serve time?

14. What is the worst thing you have ever done to a SO?

15. Have you ever put your hands on a SO for any reason other than the exception of pulling them from a burning building or shoving them from the path of a speeding train????
